Application to Neighborhood Advisory Council

This is the application that community members fill out in order to serve on the Neighborhood Advisory Council to the TN ASD. This Council is the primary reviewer of each operator’s Application to Match, and serves as the primary mechanism to collect community input about the placement decision for a new restart provider into a low performing school.

NOTE: The ASD holds a “Creative Commons Attribution” license for this resource and requests that people interested in using part or all of the documents reach out to the ASD team. People/organizations interested in editable versions of a document should submit an ASD Help Desk ticket with their request(s).

Outcomes and Lessons Learned: Community members were not especially enthusiastic to fill out these application forms and some members were challenged to turn in their completed applications. Some community members completed the application without fully understanding what it meant or what would be entailed. In the future, we plan to explain more about the application is and why we are asking people to complete it.
